POLICIES AND STANDARDS

At Integrations, new clients are required to fill out a brief medical questionaire
and sign a policies statement.

Also, if you desire a Hot Stone Massage, or as many clients have found that they
enjoy the occasional use of hot stones integrated into their standard sessions, a
“Hot Stone Consent” form must be signed for insurance purposes.

We do this on the day of your first appointment, however, to save time
on the day of your appointment, you may fill out the Policies Statement prior to your appointment and bring it with you.

You may also view if you wish, the hot stone consent form…..

.and possibly determine if the use of hot stones is something you feel you would desire – if so, you may complete the consent form and bring it with you as well OR you may wait till the day of your appointment and ask questions you may have.
